Providers should complete the Intent to Apply/Intent for Reaccreditation Form and submit the accompanying eligibility review/reaccreditation fee prior to the cycle deadline (see Timeline).
Prior to submitting an Intent to Apply form, organizations should contact Joint Accreditation Staff ([email protected]) to discuss eligibility requirements and to understand the Joint Accreditation process. Please also notify Joint Accreditation Staff in advance if your organization requires an invoice to process the Intent to Apply/Reaccreditation fee.
